Abstract:
Title: Care portal for managing patients with Diabetes. using a micro services architecture. Getting real-time-data using a prediction engine to prevent diabetes threshold.
Value proposition: Diabetes is a chronic disorder, heavily impacted by patient’s day-to-day behaviors. Meal, activity and other daily activity play an important role in glucose control and progression of the disease. Current management strategy is commonly driven through periodic (3-4 per year) visits to physician usually lasting 15-20 mins long. These care protocols are usually insufficient to uncover problematic patient behaviors and improve day to day patient behavior. Digital innovations and remote monitoring techniques can provide new ways of managing patients effectively through better patient engagement, in-the-moment feedback and sustained behavior improvement. The goal for the project is to create an care portal for remote management of patients with diabetes.
Goals: Develop care portal based on micro services architecture with following features
- Web application to remotely monitor and manage patients
- Patient facing engagement app / client / chatbot
- API connections to connect to nutrition and activity databases
- Backend services to create and deliver data driven insights using glucose, nutrition and activity data
- Deliver personalized therapy plan emails periodically for patient and physician review
