For the written portion of the midterm, will we be allowed to use our notes (i.e. lecture slides)? How about the programming portion, will we be allowed to use lecture notes, previous labs/hw and lecture code examples?

For the written part you cannot use anything besides your pen/pencil.

However, for the programming part you can use your notes, homework code, lecture code, and any other code you think will help you. Also, its closed internet with the exception of access to csns.
