related table has all information like username, first name, last name   and couple of email addresses

I am trying to know, what is the exact format of the display

1. username    |  first name | Middle Name  | last name       | email address1   |   email address 2

2 Username   |Last name    |first name    | email address1

3.  Last name    |first name    | email address1

4 Any one of the above is OK

5. None of the above is OK( Could you please suggest)
